Child Dedication Form
Are you planning on bringing your cute, beautiful/handsome baby to the alter for dedication?
That's awesome, just click the button below to download the HOTR Child dedication form in PDF format, print and fill it, then submit it to the church office for further processing.
NOTE:
(1). Dedications are done every last Sunday of the month during the First Service (unless otherwise announced by the Church).
(2). Couples are expected to be in Church by 7:30am.
(3). Pre-dedication class holds on the Last Friday of the month (unless otherwise announced by the Church).
(4). An introduction letter is to be submitted from your Pastoral Care Unit (PCU).
(5). Pay a fee of N2,500 only.
